Parental Consent: I hereby give permission for my child to attend the YMCA MSUN Assembly May 1 – 3, 2009. I understand my child will be transported by YMCA or chartered buses and that my child will be under the supervision of YMCA staff members, school advisors and volunteers. I understand that I am responsible for the actions and conduct of my child during the trip. In the event of an emergency in which I cannot be contacted, I give permission for Emergency Medical Staff and the YMCA to take appropriate action in the best interest of my child or ward.

Photography – I permit the YMCA and/or the media to use images of my child as a YMCA program participant in internal and external promotional material. This includes any printed material, broadcast and print advertising, promotional videos and the YMCA website which are produced or published by the YMCA. I also permit the YMCA to use images of my child in broadcast and print media news coverage of the YMCA. I understand that my child’s name is not published.

Code of Conduct: As a participant of YMCA NC MSUN, I acknowledge that my advisor/ teacher has reviewed the student code of conduct with me. I will follow all rules and procedures in accordance with the code, as explained to me by my advisor. I understand that I am required to attend all events. I also understand that I represent my YMCA and my school and should follow all guidelines during this program. I understand that there are two double beds per room and I will share a bed with a student of the same sex.
